Job Title: CFS Specialist

Program: Coordinated Family Supports (CFS)

Reports to: Director of Community Support Programs

Work Location(s): Contra Costa and Alameda Counties.

Job Type: Full Time

Pay Range: $22.00 to $27.00 an hour (Based on experience)

Job Summary

The Coordinated Family Supports (CFS) Specialist will identify and provide support necessary to help adults with developmental disabilities, live successfully in their family home.CFS services may include identifying community resources, providing assistance and navigating services, finding generic resources, and providing future planning and training.

Job Duties and Responsibilities:

• Identify and provide supports necessary to successfully reside in the family home.
• Provide assistance and training for the consumer and their family in navigating comprehensive services and supports that are tailored to meet their unique needs, including creating pathways to overcome barriers to accessing generic and other resources.
• Coordinate training across providers specific to the needs of the consumer and their family.
• Assisting with scheduling of service delivery including medical and other appointments.
• Identifying transportation options or services.
• Identifying backup providers/supports.
• Providing future planning for the consumer.
• Prepare and submit quarterly reports on progress of service delivery.
• Provide general administrative and support duties as assigned.

Job Qualification:

• A bachelor's degree in human services or an associate's degree in human services or three years of experience in the developmental disabilities field.
• CFS staff can communicate in the primary language of the consumer and their family.
• Valid California driver's license.
• First Aid/CPR Certified.
• Valid automobile insurance for that vehicle and all passengers that meets minimum coverage requirements of the agency (100k/300k liability/bodily injury).

Job Type: Full Time

Pay Range: $22.00 to $27.00 an hour (Based on experience)
